CAMED Legacy Care Circle: Membership Terms & Conditions
1. Membership Grades & Monthly Donation

This membership is based on a Monthly Donation model to support the non-profit mission of the CAMED Care Foundation.

Grade A & B: $29/month (Requires proof of income per Federal Poverty Guidelines)

Grade C: $59/month
-
Grade D: $99/month
-
Donations are automatically processed on the designated billing date each month using the registered payment method.
2. Core Benefits

Monthly Complimentary Session: All members receive one (1) free therapy session per month. This session must be used within the current month and does not roll over to the following month.

Discounts on Extra Sessions: After the first free session, additional sessions are available at a discounted rate based on your grade:

Grade A/B: 70% OFF

Grade D: 30% OFF

Priority Booking (Grade D Only): Grade D members receive priority scheduling to ensure access to their preferred time slots.
3. Appointment & Cancellation Policy

By Appointment Only: All services must be scheduled in advance.

24-Hour Notice: Please provide at least 24 hours' notice for any cancellations or rescheduling.

No-Show Policy: If you cancel with less than 24 hours' notice or fail to show up, the complimentary session for that month may be forfeited. This policy ensures we can provide care to other seniors on our waiting list.
4. Usage Limits & Re-evaluation

Non-Member SFS Users: To ensure fair distribution of resources, non-member low-income (Grade A/B) users are limited to 10 sessions per calendar year. Beyond this, joining a membership is highly recommended for continued care.

Eligibility Review: SFS eligibility (income verification) for Grade A & B will be re-evaluated annually or semi-annually to maintain transparency.
5. Legal Disclaimer & Tax Benefits

Non-Profit Mission: CAMED Care Foundation is a 501(c)(3) non-profit organization. Your donations directly support therapy and rehabilitation for low-income seniors in our community.

Tax Deductibility: Your monthly donation, minus the fair market value of the services received, may be tax-deductible. Please consult with your tax advisor for details.

6. Membership Cancellation

You may cancel your membership at any time. To stop the next scheduled donation, please notify the Foundation office (Sophia or Lupita) in writing or by phone at least 7 days prior to your next billing date.
